Truly, an example of D&D acting ay it's best -- as a conflict resolution system where individual actions and results take one measurably closer to a given result while producing entertaining detail. I'd probably have had actions within an initiative rank be decided by attack roll, but yours has other advantages.

Have you thought about counter-attacks, I wonder? It seems to me like the obvious way to handle them would be as a "ready an action to counterattack" action -- if the counter succeeds (ie, hits), the attack roll is considered to be your defense! But if it fails, you take a penalty to your defense (not sure--could be "lose your dex bonus", could be a fixed penalty to your AC, like -2). I'd probably want to add a few more restrictions in -- particularly, "no countering weapons more than one size category larger than yours" (can't counter a troll's pike with your rapier, nor any pike with a dagger, but making a stop hit against the troll's claws is fine) and probably "must have your dex bonus" (ok, I'd also want to throw in an interaction with feints (ie. feints trigger the readied action for a counterattack), but that's probably too much).
